Use of Services
- Services are available to adults 18 and older. Minors require legal guardian consent.
- All clinical services require an initial consultation and Good Faith Exam.
- Programs and memberships are non-transferable.
Payments
- Fees are due at the time of service unless otherwise agreed in writing.
- Memberships auto-renew monthly and may be cancelled with 30 days' written notice.
- Financing may be available via CareCredit, subject to credit approval.
Intellectual Property
- All website content, protocols and branded materials are owned by BMS Wellness MD or its licensors.
Limitation of Liability
- To the maximum extent permitted by law, BMS Wellness MD is not liable for indirect, incidental or consequential damages arising from use of this website or our services.
